American Partnership for Eosinophilic Disorders’ (APFED) mission is Education, Awareness, Support and Research of Eosinophilic Disorders.
To support it’s mission, APFED hosts an annual national meeting for patients and families affected by eosinophilic disorders. These meetings unite top researchers, patient care teams, nutritionists, psychologists and other experts in the field, all of whom volunteer their time to educate families.
About the EosConnection Travel Award Program
The EosConnection Travel Award Program seeks to encourage and support families in need to attend the 2008 patient and family conference which will be held in San Diego, CA July 11th-13th 2008. The award is to be applied towards travel expenses which include: hotel, transportation, mileage, meal and/or childcare costs.
APFED will distribute up to eight $500 awards as long as APFED receives qualified applications for these awards and sufficient monies are available to fund the awards.
Applicant Qualifications
Applicant cannot have attended an APFED conference in the past. First time attendees only.
Applicant’s and/or applicant’s child must be diagnosed with an eosinophilic disorder.
Annual family income for the $500 award should not exceed $80,000 per year.
Application Requirements
Applicants must provide the following:
A letter from a physician documenting the applicant’s or applicant’s child’s diagnosis.
Brief description of why the applicant wants to attend the conference.
A list of projected expenses with supporting documentation (i.e. transportation estimate from a travel agent, airline or travel website; number of driving miles from home to the conference location; hotel costs, estimated meal expenses). Note: Meal per diem not exceed $40.

Copy of the previous year’s W2 form or similar documentation of income. Please black out your Social Security number before emailing this document.
The applicant must not have a separate and/or different grant that covers the same expenses covered by this application.
Applicants must complete this application and submit to APFED by May 15th, 2008. Applications should be submitted by email to grants@apfed.org.
Award recipients must agree to complete a survey following the conference regarding the program
Grant Approval and Announcement
Applications will be reviewed and approved by the APFED Executive Board.
The Board will review all applications and make the final decision based on need.
Awards will be announced by June 10th, 2008 by email.
Award Payments
Payment of the approved award will be made upon receipt of projected expenses or expenses incurred. Supporting documentation may include copies of airfare, rail, public transportation, and hotel validated receipts and/or record of mileage. To receive payment, recipient should complete the Award Distribution form available on the APFED website at www.apfed.org.
Meal per diems are calculated at $40. Actual meal receipts are not required.
For mileage, simply record the travel mileage to and from your home and the conference location. Mileage consideration is based on $0.40 per mile.
Requests for stipend distribution may be made after the stipend is award is announced, and up to 30 days after the last day of the Conference. Any requests for reimbursement that are received by APFED more than 30 days following Conference conclusion will not be considered.
